What's Needed

Getting ready for your tax interview depends on the the complexity of you tax situation.  This is could take just a few minutes, or time and effort to gather the required documents.  Below are lists of general items I will need to get started:

Identification & Review (New Clients)

  • Photo ID - Federal and State law requires your photo ID 
  • Social Security Cards - For taxpayer, spouse & dependents 
  • Copies of prior year tax return (or most recent tax returns filed) 

Income Statement Forms

  • W-2s (Wage Statements)
  • 1099R (Pension, IRA & Other retirement statements)
  • 1099INT (Interest Income)
  • 1099DIV (Dividend Income)
  • 1099G (Unemployment or State Refund)
  • 1099W2G (Gambling Winnings) 
  • 1099B (Broker Summary Statements)
  • 1099S (Real Estate Sales)
  • 1099K (Payment Card/ Third Party Network Transactions 
  • Form 8949 (Stock sales/ Cryptocurrency trading) 

Health Insurance Statements

  • Federal Form 1095-A, 1095-B, 1095-C
  • CA Form 3895 Health Insurance Marketplace Statement 
  • Other State Health Insurance Disclosures 

Deductible Expense Statements

  • Form 1098 (Mortgage Interest Statement)
  • Form 1098T (Tuition & Fee Payments for College/ Continuing Education)
  • 1098E (Student Loan Interest Payments) 
  • Child Care Records/ Summary of Payments (Need: Name, Address, Tax ID: EIN or Social Security number of provider, total amount paid for year)
  • Charitable contributions receipts/ statements
